Abstract

 Achieving seamless data interoperability across multiple healthcare institutions remains a pressing challenge in modern health information systems. Disparate electronic health record (EHR) platforms, varied coding standards, and institutional data silos hinder unified patient care and collaborative research. This paper explores key barriers to interoperability, presents findings from prior literature, and proposes a structured overview of challenges that obstruct cross-institutional data exchange. Emphasis is placed on semantic discrepancies, technical heterogeneity, privacy constraints, and governance limitations
